Re-formatted documents

• A corrected rendering of the W3C’s very useful URNs, Namespaces and Registries report that has inexplicably had a formatting error for many years that causes half the document to be rendered in fixed-width font. This local copy fixes that closing <code/> tag, with no other changes.
